Forms 2 & 3 must be printed and delivered in hard copy to the CHS School Nurse. Please drop off at Chatham HS front entrance Monday - Friday. There is a bin labeled "Sports Physicals."  All students planning to participate in fall sports for the 2024-2025 school year must submit all paper forms by August 1. (Football Athletes must submit by July 1st)

1) Activity Fee: In accordance with Board policy #2436, students who participate in school-sponsored co-curricular activities are subject to a participation fee. The fee is paid only once per year; a student who has paid the fee may participate in as many athletic/extracurricular activities during the course of the school year as they would like without making additional activity fee payment. Please follow this link to the policy. Any student participating in Chatham High School athletics is subject to the activity fee. Click here to pay the registration fee.

2) Sports Physical Instructions - Directions on how to obtain full medical clearance in preparation for an athletic season.

3) Health History Update Form (IF YOUR CHILD HAS A VALID PHYSICAL ON FILE) or Athletic Pre-Participation Physical Examination Form (IF YOUR CHILD'S PHYSICAL IS EXPIRED): Physicals are valid for 365 days. You may view the date of your child's last physical by visiting the Athletics tab within the Parent Portal. The Healthy History Update Form must be submitted each athletic season when participating - Fall, Winter, Spring). 

4) Impact Testing Consent Form: Impact testing is a baseline concussion assessment that all student-athletes will participate in every two years.

5) Digitally Sign Electronic Forms: The following forms are available in the Parent Portal. To sign the forms electronically, sign into the Parent Portal, select the "Forms" tab, and select "Athletic Release [YEAR]." Check off all of the check boxes that apply.  DO NOT submit these forms in hard copy if you have signed off on them in the Parent Portal.

5) Review NJSIAA List of Banned Substances: Each year, NJSIAA updates the list of Banned Substances. Please ensure you are aware of the banned substances by clicking on this link:  22-23 NJSIAA Banned Substances 

Your child WILL NOT be able to participate unless every form is filled out and submitted properly.
